Built for adults with a date – or a long wait – and a blank page

Adult ADHD assessment is a clinical interview about your life, not a quiz you can cram for. People often leave feeling they went blank, gave vague answers, or forgot the childhood examples that would have made the picture clearer.

This workbook helps you answer one question: can I give a clinician a usable, honest account of my history, current difficulties, examples and questions?

What’s inside the workbook

A practical 49-page file for adults in the UK, United States, Australia and Canada.

History

Document hunt, informant pack and life timeline – including when records are missing.

Examples

Concrete childhood and adult examples across settings – not a rehearsed diagnosis script.

Questions

What to ask on the day, plus a one-page summary you can take or send ahead.

Also covered: what a credible adult assessment includes · why capable adults under-report · masking and the cost of coping · honesty sheets for other explanations · what to do if you go blank. Two speeds – an Essential Pack for a few evenings, then optional depth.

Educational preparation – nothing more

This is not an ADHD assessment, screening tool or individual medical advice. It does not tell you whether you have ADHD, and it is not designed to help you obtain a diagnosis.

A good assessment may confirm ADHD, identify something else, or leave a mixed picture that still needs follow-up. Organised history helps either way.

About Dr Suraj Gogoi

I’m an independent psychiatrist and adult ADHD clinician based in the UK. My work includes adult ADHD assessment, neurodevelopmental psychiatry and medication management.

I created this workbook because the quality of an assessment depends partly on the quality of the history. Many capable adults arrive with a strong feeling that something is wrong, and almost no organised examples.

“The aim is simple – to help you walk in with your life organised, not a script for a diagnosis.”

Questions before you buy

Is this an ADHD assessment or medical advice?
No. It is educational preparation – worksheets and guidance to help you organise your history before a real assessment. It is not a diagnosis, prescription or substitute for personal medical advice.
Do I need an appointment booked already?
No. It helps whether you have a date, are on a long wait, or are gathering material while you decide. The Essential Pack is designed for a few evenings; the full file adds depth if you want it.
How is this different from the Decision Kit?
The Decision Kit (£29) is for adults in England deciding whether and how to pursue assessment – pathways, costs and shared care. This workbook (£19) is for organising your history and examples before any credible adult assessment, in the UK, US, Australia or Canada.
Will this help me “pass” an assessment?
No. It helps you give an honest, organised account. Clinicians look for consistency across history, examples and impairment – not rehearsed answers.
How do I receive the workbook?
After purchase you get digital access to the PDF. Print the worksheets you need, or work through them on screen.

Still working out what may be going on?

If you are not ready to look at private ADHD assessment pathways, a free educational quiz can help you notice what may be contributing to overwhelm, low mood, poor concentration or feeling stuck. It is not a diagnosis or a clinical assessment.
